Enrolling in preschool means parents have forms, forms, forms to complete. Some of these are required state forms and some of these forms are those that will help Pioneer Preschool to keep your child happy and safe.

If you enroll your child at Pioneer Preschool, you will be given all of these forms when you register or at Parent’s Night. If you would like to review these forms or get a head start on their completion you will find the following forms for your use:

Enrollment form – To access this form, please click here STATE OF OHIO FORMS to go to the state form site. Enter 01234 into the form # box to view the enrollment form. Complete this form on your computer or by hand by filling out every line or using n/a when appropriate. Print the form and mail it or deliver it to Pioneer Preschool. There is no link to submit the completed form directly to Pioneer Preschool.

Medical form – To access this form, please click here STATE OF OHIO FORMS to go to the state form site. Enter form number 01305 into the form # box. This form must be printed, then signed and dated by the doctor and returned to Pioneer Preschool by the time school starts.
